Fill out the details on the form, including your signature. Keep a copy of the form for any further clarification
The form requires your family doctor's signature, as well as the signature of your optometrist or eye doctor on the vision test section
Send the completed form to: The Update and Control Department License Bureau Ministry of Transport and Road Safety P.O.Box 270 Holon 58102

| Must I undergo a medical examination to renew my driving license? |
Only license holders who are 65 years old or more are required to undergo a medical examination before renewing their driving license, and present every two years a medical examination form called "רש-ל 54" signed by a doctor, including a vision test section signed by an optometrist or an eye doctor. In addition, some license holders, as detailed below, are also required to undergo the same examinations if they are 50 years old or more:
Public transport vehicle drivers.
Heavy goods vehicle drivers.
Fire-fighting vehicle drivers.
Children transport vehicle drivers.
Ambulance drivers. |

| When do I receive the medical examination form, and how? |
The medical examination form (in Hebrew) will be sent to your home address by the License Bureau, approximately six months before the license renewal date. If you haven't received the form:
Download the appropriate (Hebrew) form: Medical Examination Form for holders of categories A, B license, Medical Examination Form for holders of category C license
To receive the form by post, contact the Ministry of Transport and Road Safety's National call centre - *5678 or 1-222-5678 from 7:30-20:00.
Please note: The examination must be done by an Israeli doctor, and cannot be done overseas. |
